Eligibility Card

  • Available to all UNINSURED Seminole County Residents
  • Available for Primary Care services WITHIN the Seminole County Heath Department Buildings
  • Clinic Card is NOT valid for services provided OUTSIDE the Seminole County Heath Department Buildings

Persons & Services Eligible for Clinic Card

  • Children (Under the age of 18)
    • Family Planning
    • Communicable Diseases Control
    • Acute Care (Must have denial letter from Medicaid. Denial can NOT be for failure to follow through on eligibility process.)
    • Maternity Services
    • Well baby & Well child
    • Pharmacy & Lab relating to Family Planning, Communicable Disease, Acute Care, Well baby, Well child and Maternity Services.
    • Medication (Epilepsy, Diabetes and Rheumatic Fever)
  • Women
    • Family Planning
    • Communicable Diseases Control
    • Maternity Services (See Section 4)
    • Pharmacy & Lab relating to Family Planning, Communicable Disease and Maternity Services.
    • Medication (Epilepsy, Diabetes and Rheumatic Fever
  • Men
    • Family Planning
    • Communicable Diseases Control
    • Pharmacy & Lab relating to Family Planning and Communicable Disease.
    • Medication (Epilepsy and/or Diabetes)
  • Maternity Services
    • A clinic card will be issued during Presumptive Eligibility for Pregnant Women (PEPW) application processing and given to our designated customer service representative. Clinic cards will NOT be issued to prenatal clients until their PEPW expires.
    • Our customer service representative will give clients issued clinic cards after Medicaid is denied or PEPW expires.
    • Our customer service representative will retrieve ALL clinic cards of Medicaid APPROVED clients and return to Eligibility staff

The Department also provides referrals for Medicaid and KidCare programs.
