Enrollment Requirements

  • Social Security card (if you have a number)
  • Proof of income (three months worth of pay stubs or letter support from supporter to include name address and phone number
  • Proof of assets and checking and/or savings account for six months
  • Proof of insurance benefits status or documentation of insurance benefits
  • Proof of positive HIV status
  • Current viral load and CD4 results (less than 4 months old)
  • Proof of residency (lease, mortgage, utility bill)
  • Prescriptions from a Florida physician with five refills

Before referring to ADAP, availability of insurance, eligibility for VA benefits or eligibility for Medicare or Medicaid must be explored.

Once the information is available, an appointment for ADAP enrollment will be made. ADAP enrollment is done on a face to face basis. Monthly medication pickups must be done by the enrollee. Re-enrollment is done every six months and the above information must be presented.
