PRISM - Patient Reporting Investigating Surveillance ManagerSurveillance

Surveillance is responsible for verifying the diagnosis and treatment of all reportable STDs from public and private health care providers. Private and public hospitals, laboratories, physicians and other health related care providers, as required by law, report positive STDs such as syphilis and HIV to the SCHD on a daily basis.

If you are not sure which STDs are reportable, or to get a copy of the reporting form click on to the Disease Control section.

After cases are reported to the State Health Office, they are forwarded to the Centers for Disease Control and Prevention.

Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public-records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.

For media inquiries please contact the Health Promotion and Public Information office at 407-665-3384. For information regarding services please call our Main Number (407) 665-3000.
